pick a day :-p ask anyone, i have these moments constantly :-p
most recent was just yesterday, fully bonded a beacon in CoF, ran into mob, died of course
attempted to res just as the enemies moved toward the hero, it died too
then i tried with another hero, without bothering to wait for them to leave....that ended badly as well
so we all ressed at the shrine
and once again the beacon was fully bonded, and i died, of course
it was at that point that i decided i should probably do something else

Wow. I've been known to bond a beacon, but I usually only do it once in a run

I tried Sanctum Cay HM 5 times before I realized it would be a good idea to try to pull the mobs at the first bridge instead of fighting them all at once (that's something like quadruple aggro there).
I tend toward lots of little things, that I do sometimes 8 or 10 (or more for build errors) in a zone.
- Trying to daze warriors and rangers (there may be occasions when that could help, but the PvE situations I've been in it really doesn't).
- Switching characters and trying to use the new char's build like whatever Pantheo's running at the moment (like trying to use BHA when it's actually wild throw in that spot); this is worse than switching builds on Pantheo and trying to use it like another.
- Forgetting to turn on HM and dying in the first mob (as a 600).
- Forgetting to put Kitty back on guard when we go to fight again and wonder why it's harder to kill things (and occasionally it means the difference between winning and losing >_<).
- Getting between two difficult patrols (when I know they're there).
- Bringing an MM in zones with no corpses.
- Bringing a fire ele along in Ring of Fire.
- Forgetting to swap weapons back (especially when enchantment length is critical, or when that +5 energy my main bow has makes a difference).
- Forgetting to unflag heroes.
- Flagging heroes right into mobs.

I was vanquishing in Morostav Trail yesterday using the triple necro setup. I forgot that I had swapped out Livia, my healer necro, for Zhed, so I could get an item off of him. I went in with the SS, MM, and Zhed instead of the healer. So the only one healing me was Danika... I vanquished 100 foes before I noticed that Livia wasn't in my party.
I figured I might as well continue on, and I managed to finish the run with just 1 healer.
Also, I couldn't tell you how many times I've put pain inverter on a monk...

One of the dumbest things I've done is right after my henchies quadrupled aggro in HM, back before PvE skills and the like, I flagged them away to try and escape. I then tried to fight them off long enough for the H/H to escape, and forgot to run as well, was surrounded completely. 
I also tend to use prep shot without a prep on at the beginning of the fight. I hate when I do that.
As for PI'ing a monk, I do that, but on purpose. If others have PI in my party, I figure in HM a monk can actually wand for a reasonable amount, so it does a nice bit of damage.

Fortunately I don't do it accidentally too much ('specially lately, carrying PI and BHA is quite handy), but purposely only works if you actually spend time in groups with others >_<. Sometimes I have 'fat finger syndrome' and use BHA (2) and PI (1) on the same target. Or I forget how my skills queue (heroes and Kitty queue differently >_<) and try to get it to swap without canceling whatever I hit first. It's not a complete loss, 'cause of the wanding, but it means that enemy takes longer to die, and I may not be able to use either on the next target right away. (Fortunately one person isn't usually the target of all the hexes and such in a full party >_<; forgot spellbreaker once and had 7 or 8 hexes on me.)
Also like trying to kill a mob without realizing there's two or three monks, and we spend all our energy on the first enemy (or two if we get lucky there). And of course they're usually really big mobs, so not only is our energy gone, but we tend to wipe eventually.
